Summary
The CAFP is seeking a part-time Member Engagement & Outreach Coordinator to support member experience, retention, and outreach. This role serves as a key point of contact for CAFP members and helps ensure members feel connected, supported, and engaged with the Academy鈥檚 work.

This position is well-suited for someone who is highly organized, responsive, warm and professional in communication, and comfortable learning and working within established systems. The ideal candidate enjoys relationship building, values follow-through, and thrives in a small, collaborative nonprofit environment dedicated to supporting Colorado鈥檚 family physicians and their patients.

The Resident Experience Assistant is responsible to for assisting with programming, encouraging
participation, and promoting activities on the monthly calendar created by the Resident Experience
Director. The Resident Experience Assistant will assist the Resident Experience Director in set up
and break down of activities and events, coordinate updates or changes of Life Story Profiles, support
outings, maintain inventory of supplies and assist in calendar or newsletter creation as requested. All

The Specialist, Prevention Navigator provides intensive care coordination, support, and services to people who inject drugs, many of whom are already clients of Vivent Health鈥檚 LifePoint Program, which provides syringe access services. The goals of Prevention Navigation are to help clients reduce barriers and build the skills needed to access services such as medical insurance, hepatitis C care, and addiction treatment services for those who are ready. The Specialist, Prevention Navigator will implement a client-centered approach rooted in harm reduction. This position will work with clients to develop an individualized plan to access prevention and treatment services for a broad range of health needs identified by the client.

In addition to providing service coordination, the Specialist, Prevention Navigator will be instrumental in identifying, building, and expanding existing relationships with relevant community-based organizations and health centers to create a regional prevention referral network. The Specialist, Prevention Navigator will ensure ongoing network partner engagement by working closely with organizations to understand their capacity to take referrals and establish points of contact with key personnel.

The Division of Geriatrics is seeking a compassionate social worker to join our multidisciplinary team in a geriatrics. This provider will help lead the development and implementation of a new elder abuse intervention program, embedded into primary care.听听This is a grant funded position and will include direct clinical care, administrative duties, and participation in research activities.

This integrated care management and behavioral health position will provide comprehensive care coordination, resource navigation, and evidenced-based interventions to older adults experiencing mistreatment and their families or support networks. This position will work in close collaboration with the medical team, including geriatricians, clinical pharmacists, nurses, medical assistants, and care managers. This position will also include outreach work with adult protective services, law enforcement, and other community providers.

This is a 0.5-1.0 FTE position, including actively participating in Division of Geriatrics meetings, clinical care conferences, and university service/committee work.

Provide assessment and/or ongoing social casework services to families where child abuse, neglect, or youth in conflict issues are present or suspected. Caseworkers provide 24-hour response to reports of abuse and/or neglect in accordance with guidelines set forth in Federal and State law including the Colorado Children鈥檚 Code and Colorado Code or Regulations Volume VII. Develop case plans to address issues impacting the safety and well-being of children and youth. Make referrals to other agencies, when appropriate. Receives supervision from the unit supervisor, program administrator and/or director in the assigned area.

This position is responsible for supporting the legal program, which helps ACC鈥檚 clients in obtaining asylum, parole, work permits, residency, and other immigration benefits. Your main focus will be on coordinating legal appointments, collecting and organizing needed documents, serving as a liaison between organizations and clients, utilizing interpreters to communicate with clients, providing data keeping and filing, and preparing fund requests as needed. Flexibility, organization, ability to multitask, ability to take direction, and good people skills are key to succeeding in this position. Native Spanish speaker desired. Spanish fluency a must.

The Family Coach partners with families to identify opportunities, set meaningful goals, and support positive changes that strengthen overall stability.

A core expectation is to proactively reach out, engage families in program spaces, and initiate conversations that create pathways for participation in coaching and supportive programs.

The Action Center is developing a dynamic community center in a repurposed elementary school, bringing together health and supportive services from multiple organizations under one roof. Through strong leadership and partnership, a coordinated system of care will provide comprehensive, collaborative services that empower the community to build healthier, more stable lives. The Action Center expects to move into this new space in 2027.
